当肩胛骨外科颈断裂合并体部粉碎性骨折时 ， 在侧位片X线上肩胛冈移位可表现为“Z”样畸形 ， 从而导致肩胛骨坍塌以及肩袖损伤 。
黄长明8根据对FSI和SSSC概念及其损伤机制与治疗选择的理解 ， 提出将FSI分为3型损伤 ， 分别为: 。

8、A型 ， 同侧肩胛颈骨折+锁骨干骨折；B型 ， 同侧肩胛颈骨折+锁骨外侧端骨折；C型 ， 同侧肩胛颈骨折+肩锁关节脱位 。
3.3 治疗的选择 对浮肩损伤的治疗有争议 ， 无论手术治疗还是非手术治疗均有疗效优良的报道 。
Edwards 等5对20例浮肩损伤患者进行非手术治疗 ， 仅1例枪伤导致大块骨缺损的锁骨未愈合 ， 其余骨折均愈合 ， 肩关节功能良好 。
他们认为对骨折移位 本组FSI手术取得了满意疗效 。
我们认为 ， 手术固定可允许患肩的早期功能锻炼 ， 因而能减少创伤后冻结肩的发生 ， 此外 ， 浮肩损伤常伴有其他部位的损伤 ， 此时手术治疗也有利于伴发损伤的诊治和护理 。
浮肩损伤使维持肩关节的稳定结构破坏 ， 对于此类不稳定型FSI ， 进行手术治疗 。

9、是必要的 。
手术成功的关键是肩胛颈骨折的解剖复位 。
当肩胛颈骨折合并锁骨远端骨折或肩锁关节脱位 ， 拟行钩钢板内固定时 ， 首先应整复固定肩胛颈骨折 。
因钩钢板对肩峰后方的过度提升或抬起不足 ， 将妨碍肩胛颈骨折的准确复位 。
术后指导患者循序渐进行康复训练 ， 降低各种并发症的发生 ， 尽早恢复肩关节功能 。
